Foundations helps young people (aged 21 to 28 years) who live with a developmental disability transition from high school to living on their own.

The program is designed to assist young people with their independence and to build life skills.

This service is offered in Kawartha Lakes and Haliburton.The Foundations program helps young people to:

  • Plan for their transition into adulthood.
  • Connect to their community by participating in various activities of their choosing.
  • Build on existing life skills and enhance healthy living practices.
  • Develop relationships and work on social skill development.
  • Support volunteer or employment activities and enhance job-readiness skills where needed.

Foundations offers life and work skills training on a one-to-one basis and in groups. Through skills training, it helps young people to:

  • Take care of themselves and their homes.
  • Manage their money.
  • Develop self-awareness and self-confidence.
  • Enhance their decision-making capabilities.
  • Develop communication and assertiveness skills.
  • Learn organizational skills.
  • Plan and prepare healthy meals.
  • Know their community.
  • Take part in recreational and leisure activities.
  • Connect to community partners who can assist with housing, financial supports, etc.

The Foundations program is funded through MCCSS with referrals coming through Developmental Services Ontario (DSO). For more information, please contact the DSO at www.dsontario.ca and let them know that you are interested in supports from Community Living Trent Highlands.
