Description (Service)

Family and Youth Services - provides support to families with a child, 3-18 years, diagnosed with a developmental disability.

Family Support Services work with families as they navigate life in the community. Offering support to access resources that meet the immediate and future needs of the family.

Services include:

information on developmental disabilities, community resources and financial assistance
application to Special Services at Home (SSAH), Assistance for Children with Severe Disabilities (ACSD) or alternate funding sources
referrals to appropriate resources
co-ordination of services, advocacy and transition planning
ongoing communication with involved agencies and schools
respite supports

Hours	Office Mon-Fri 8:30 am-4 pm
Areas Served	Parry Sound District ; Parry Sound District - West (includes Dunchurch)
Eligibility	Youth 3-18 (up to 21 if still in school) years who exhibit significant delays in two or more areas of development such as intellectual, psychological, social or motor.
