Description (Service) A team of health care professionals focused on health promotion, illness prevention and primary health care services.

Team Includes:

Programs offered include:
  • Healthy Eating to Manage Chronic Disease workshops
  • falls screening clinic
  • lactation program
  • lower extremity strengthening & balance
  • PACE fitness counselling
  • cognitive screening
  • chronic disease self management
  • diabetes management team
  • cardiovascular/hypertension home monitoring
  • COPD
  • tobacco reduction program
  • planned senior's health day program
The CCFHT provides after hours care at four locations in Gravenhurst and Bracebridge and Port Carling on a rotating basis for patients who are rostered with the team
  • if you are a registered patient and require non-emergency medical care, you can be seen at the after hours clinic at any of these locations
  • phone your own doctor's office and a message will advise as to which office is open on that day
All physicians have hospital privileges at Muskoka Algonquin Healthcare - South Muskoka Memorial Hospital Site

Hours Mon-Fri 9 am-4:30 pm After hours clinic Mon-Fri 5 pm-8 pm (rotating sites, by appointment only)
