Description (Service)

Support for people living with pre-diabetes or diabetes. Support for pregnancy with type 1 or type 2 diabetes and gestational diabetes. One-on-one or group consultation on monitoring blood sugar levels, healthy eating, how to administer insulin and use insulin pumps. Registered nurse, registered dietitian and Certified Diabetic Educator (CDE) on staff.

Periodic education and awareness seminars and workshops are offered to the general public in the community.

Eligibility Adults and seniors with diabetes (Type 1 and Type 2) and gestational diabetes

18 years and older
Application Call to make an appointment or referral from physician, registered nurse, health worker referral (available via website) or self referral.
