| Description (Service) | Provides temporary health services to patients who DO NOT have a family doctor or nurse practitioner, while they wait to be connected to a local family physician. Patients at this clinic must be residents of Collingwood, Clearview Township, Wasaga Beach or have a L9Y Blue Mountain postal code. Patients must also register for Health Care Connect, the provincial waitlist for a family doctor (https://www.ontario.ca/page/find-family-doctor-or-nurse-practitioner). When a family doctor becomes available, Shoreline patients will be placed with that doctor. Once a permanent provider becomes available, the patient will no longer have access to care from the Shoreline Health Clinic. Services at this clinic are covered by OHIP and are provided at no cost. To register for this clinic you must complete an intake form. Please follow the process outlined in the application section. |

| Eligibility | Patients who live in Collingwood, Clearview, Wasaga Beach and those in The Blue Mountains with L9Y postal code and do not have access to a family doctor or nurse practitioner anywhere in Ontario. Patients with a physician anywhere in Ontario are unable to access service at the Shoreline Health Clinic. |
